Any sign-up after the 2nd sign-up will be considered a late sign-up. Any and all late sign-ups will be placed on a waiting list. Should the late sign-up be placed on a team the late sign-up will be accessed a $20 fee plus all applicable fees.

In consideration of my child or ward participating in a sports program sponsored by Delhi Hills Athletic Association, inc. (the Association) I / We acknowledge that we will be considered as members of the Association and as members of the Association, I/We do hereby release and forever discharge any and all volunteers, coaches, sponsors or organizers of the Association from any and all claims, demands, damages, actions or causes whether on account of damage to bodily injuries, including death, or property resulting or to result from any accident arising from my child's participation in the sporting activities sponsored by the Association. I/We further agree to indemnify, to forever hold harmless and to defend and assume the costs of defense of any and all volunteers, coaches, sponsors or organizers of the sporting activities by the Association. I /We acknowledge and agree that the Association does not provide any medical or hospital insurance coverage for any injuries that may be sustained by my child and I/we shall be responsible for any and all medical costs and/or other expenses incurred as a result of any injury. I/We further acknowledge that my child's participation in any sporting activites sponsored by the Association is considereed as a sport and/or recreational activitiy per the laws of the State of Ohio. I certify that I/We have read the above and fully understand the term and further agree that by continueing on that represents the rights of any other parent or guardian as if they were signing the Liability Release and Waiver.
